You must live in Shelby County, Kentucky, or if you are a member of the following libraries, you may also obtain a card:
We encourage parents to bring their kids to the library, and we want your child to have their own library card! There’s no age requirement to obtain a library card, but children 16 years or younger must have a parent or guardian agree to be the responsible party for the materials borrowed.
Complete the form below and a staff member will create your library card account, usually within 5 business days. We will contact you when your card is ready. If you have us mail your library card to you, we will process your request within 5 business days, but we cannot guarantee when your card will be delivered through the postal service.
Because we are required to see proof of address, please show your ID or a piece of mail that matches your ID to staff next time you’re at the library so we can verify your information.
All fields are required, except when otherwise noted.
We can make you a new one! Just visit the main desk on the third floor with your picture ID. We are temporarily pausing the collection of replacement card fees.
Stop by the front desk at the library to get your library card, and in a few minutes you’ll have access to thousands of books, computers, over 20 online resources, and more!
